Is it beneficial to file two dental insurance claims together?

Embed Claim in my dental insurance claim?
I am allowed to two teeth cleaning per year with no six month interval requirement which the insurance customer service reps confirmed at least on two occasions. However, after getting my 2nd teeth cleaning for the year, they dug out some mysterious " EMBEDDED CLAIM" for some service rendered back in January of this year. They claim that EMBEDDED CLAIM contained another teeth claim so I can't have 3rd teeth cleaning covered. They added that EMBED CLAIMS do not show up online on my insurance account. Had they shown the EMBEDDED CLAIM online I would not have gone for the cleaning, and I seriously have doubts about embedded claim as I never received the EOB.However, the submitted cost for the cleaning is $80. Do I just swallow it and pay because it may not be worth the process of appealing and writing bunch of letters to BBB and making phone calls and keep explaining? There was no mis communication. The insurance company's customer service reps clearly misinformed me, and they should have listed this embedded claim online under my account. What do you think I should do?It sounded like " embedded claim" is a sequential claim to another claim, but I have no recollection of receiving treatment the insurance mentions I received based on " embedded claim" . Back then I was with another dentist, and now I'm with a new dentist so make it a little more complicated

Question about dental insurance claim?
My family and I went to see the dentist last month since my mom bought the dental insurance however, after the whole family had done the service, the clerk at the dental office finally found out that our insurance was not active. Now, they are charging us $3K total for everything. So, I wonder whether the clerk should check the insurance beforehand or do they usually check it after it's done? Thank You

Dental insurance claim?
I dont have dental insurance, but i recently got some work done that costs 1500 because I didnt have the money right away, they set up a payment plan through wellsfargo CARE. its for a cleaning and my wisdom teeth being taken out and cavities. So if i apply to an insurance company like blue cross, would they cover the 1500? I only want to get insurance to pay for the work that was done 3 weeks ago, then cancel it because my dentist only charges 40 bucks for xray and cleaning so I really have no use for it except for the 1500.

Who do I write to in Texas to file a complaint re: dental insurance claim insurer won't pay - it's a hassle!
